About me
Fitness has always been important to me. Throughout my childhood it was my dad that got me moving. Whether it was mowing the lawn, going for long hikes or bike rides, he was the one who inspired me to be where I am today. Sometimes we need a push from another person to maintain our drive and get us on our feet each day. Today, I continue the hobbies that got me moving. I love to waterski, barefoot ski, hike and work out to maintain my overall fitness.
I want to be the one to inspire you like my dad did me. I want to be the one to push YOU. As your personal trianer, I will be here to motivate and educate you to be successful. From flexibility and core strength to functional training and bootcamps, I will help you achieve YOUR goals. Overall health and well-being require dedication and commitment. It’s a lifestyle.
After attending a four-year program at the University of Wisconsin-Milwaukee, I love that I continue to learn while I teach others. With a Bachelor of Science in Kinesiology (the study of human movement sciences), it is rewarding to be able to apply my knowlege and help others reap the rewards.

About Me
I have been in a health and fitness setting for most of my life. Playing 3 sports through high school, up to college, and my parents running their own fitness facility for over 10 years, it was almost inevitable that I would become passionate about making a career out of the exercise science field. That being said, I have had very rewarding experiences working with the general population who want to shed a few pounds, improve their overall quality of life, move around pain free again, or improve their physique, as well as, working with athletes who want to become more successful in their sport.
Throughout my years in the gym and working with other fitness professionals, I have found and learned that consistency, self discipline, and balance is THE secret to reaching and sustaining any and all goals. My job will be to broaden your knowledge, help you stay consistent, remain injury free, but most importantly, to ENJOY THE PROCESS.

About me
Exercise IS for everyone. No, it is not easy, if it were, everyone would be healthy and happy. But it’s not impossible, or no one would be. Exercise is not limited to lifting weights or running on a treadmill for hours, options for exercise are limitless. It is structured play and can be fun! I have a degree in Kinesiology and Psychology, with a focus on Exercise Physiology, Biomechanics, and Nutrition. I am also an ACSM certified personal trainer. I have 3 years of personal, group, and virtual training. Long story short I can confidently say I know what I am doing. Beyond the science, my philosophy of exercise is that it can and will be life changing if you let it. My goal as a trainer is to not only be an example of a life changing transformation, but also teach you to be your own trainer.
I will help you achieve your goals and teach you how to train yourself along the way. My goal is not to be a need for you to excel, but a luxury to go beyond what you can do on your own. Anything is better than nothing, future you will thank you.

About me
Health, fitness, and overall wellness have been my focus for the past 8 years, but fitness has been a huge part of my life since I was very young.
It all started with my mom and dad instilling the joy of sports into mine and my brothers’ lives. Around six or seven years old, my brother Erik and I began to see the importance of strength and fitness and how it directly applied to how we performed in sports. We even created our own (very basic) workout program consisting of mostly pushups, pull-ups, sit-ups, running, and hundreds of bicep curls. Before we knew it, we were maxing out some of our school’s physical fitness tests. Comical, yet amazing to reflect on how far back fitness and health goes for me! Fast forward 12 years and Erik introduced me to the sport of CrossFit.
This sparked a fire inside of me that spurred a whirlwind of fascination in health and wellness. That fire in my heart constantly pushes me to pursue excellence in everything I do, especially nutrition, mental resilience, and fitness. With all of that said, these reasons are exactly why I am SO excited to be a trainer and coach at Form and Fitness, surrounded by an amazing team of elite-level trainers, coaches, therapists, and staff.
Prioritizing health and fitness improved my life to levels I did not know existed. Now, it is my dream to help as many people as I possibly can; showing and leading by example, how dialing in nutrition and exercise can produce the best version of yourself.
See you out there!

About me
The health and wellness field has always been of interest of mine dating back to Freshman year of High school. I joined my first gym in 1998 where I began lifting weights and became interested in how the human body works.
While I have been working out regularly since then, I actually began my fitness career working as a Membership Lead at the front desk in 2017. I loved my experience at the desk as it gave me an opportunity to motivate, encourage, and inspire new and existing members.
Realizing my love for fitness and motivating people, I knew it was time to further my career by becoming a Personal Trainer. I am proud to say I am now an ISSA Certified Personal Trainer, and cannot wait to change your life. My areas of expertise include: weight-loss, muscle hypertrophy, functional fitness, and nutrition.

About me
Exercise has always been a part of my life and I’d love to make it part of YOURS! From soccer and ice hockey in high school and college, to yoga and Tai Chi later on, I’ve never stopped moving. I love to work out, and I want all of my clients to feel what it’s like to have MORE energy, MORE strength and MORE flexibility at any age. I will help you achieve this with functional exercises, strength training and plenty of core and balance moves. I always try to keep the workouts challenging and fun. I am an NASM certified personal trainer.
In addition to training, I work out and I still play ice hockey. When I’m not running or skating around, I may be found cooking, reading or just hanging out with my husband and three children.

About me
Fitness became a passion for me once I finally conquered my exercise-induced asthma. After 23 years of having that as a crutch, even in my everyday life, I dedicated myself to getting it under control.
After that, everything else followed suit; my health became a priority as well as helping others reach their goals. I make it a goal not only for people to have a challenging workout, but also to make my training sessions as fun and entertaining as possible. I enjoy running Spartan Races with my wife, who is a Zumba instructor here at Form and Fitness.
Fun Fact:
I’m an actor, so leading groups through exercise is as enthralling to me as being on stage in the spotlight!

While I have played sports my entire life, and still play softball to this day, my serious passion for health, wellness, and fitness began a few years ago when my friend encouraged me to start working out. I also pursued my NASM Certified Personal Training License while finishing school.
I started my career in personal training during my final year at UW-Whitewater working with several different clients in accomplishing all their different goals.
I also completed the Form and Fitness Internship/Certification Course, which is how I came to work at Form and Fitness as a full-time trainer once I graduated.
Now I am working the job of my dreams as a personal trainer and helping people accomplish their goals! Building confidence, comfortability, and knowledge in people to help better their relationship with exercise and health is my specialty, along with all other goals and challenges you bring to me! About Me
Athletics has always been a huge part of my life from playing soccer at a young age all the way into college. It wasn’t until I started my bachelor’s degree in exercise physiology that I started to understand the real importance of exercise and the impact it has on not just your body, but your overall health.
I found my place at Form & Fitness by completing the internship program at the end of my senior year and was able to experience how exercise affects your everyday life. It was then that I really started my own fitness journey and saw the positive impacts it had not only on my physical fitness, but my mental health as well.
My goal with clients is to help them not only achieve their goals, but to also focus on functional fitness to help support their everyday life. I believe that starting with the basics of movement is key to help you really achieve your fitness goals and get you to where you want to be!

Logan Hughes
I graduated from Concordia University of Wisconsin in 2021 with a major in Exercise Physiology. I’m CSCS certified through the National Strength and Conditioning Association and also hold a certification from the Parisi Speed School. Growing up as an athlete since the age of six, this field slowly became engrained in me.
For over five years now, I’ve been a personal trainer, working with a wide range of clients, each with unique goals and challenges. I’ve had the privilege of working with kids as young as seven just beginning to understand their bodies, professional athletes working to enhance their performance, high school teens, busy moms and dads, as well as gaining practical experience working with the elderly.
I thrive on creating programs that work for my clients’ lifestyles and specific goals. I also love working hands-on, in person, and watching my clients grow into more confident and stronger versions of themselves.
Brielle Hulick
My journey into health and fitness started uniquely: with a childhood diagnosis of rheumatoid arthritis. My doctor’s advice—stay as active as possible, for as long as possible—became a guiding principle. This deep-rooted understanding of movement’s power has shaped my approach through every season of life, from navigating pregnancy and postpartum recovery to overcoming injuries. I’ve learned that adaptability, consistency, and gratitude are the true pillars of sustainable health.
As a runner and cyclist, a strength training enthusiast, and a Taekwondo student, I thrive on discovering new ways to move. My decade of experience in dietetics complements my ACE Health Coach certification, allowing me to offer holistic support that goes beyond just exercise. As a mom of two boys, I understand the demands of real life, and I’m passionate about helping you discover how physical activity can truly enhance your overall well-being. When I’m not in the gym, you’ll find me recharging on or near the water.
Nadia Frohmader
About me
Being active has always been a huge part of my life beginning with gymnastics at a young age and into high school and then powerlifting. I found my passion for fitness and health through powerlifting while moving through the three main lifts: squat, bench, and deadlift. I was fascinated by how much weight the human body could truly lift with proper training and form. In high school, I always enjoyed beating the boys in pull-ups and lifting heavier weight than them.
I believe in empowering every individual to build a sustainable and healthy lifestyle. Not only through consistency but education as well. I focus on individualized programming with both physical and mental well-being in mind. My goal is to create a positive environment allowing clients to gain confidence, remain motivated, and maintain a strong mindset through growth all while in a gym setting.
I look forward to working with you soon!

Don't simply
take it from us!
Form & Fitness is the #1 personal training gym in Wisconsin! Don’t just take our word for it… check out what hundreds of our amazing clients have to say about us!
- Energy and visual is nice. If you like to get some work done at the gym there isn’t a quiet space to do so, it is more of a café setting seating area with louder music.
-Pictures posted on here haven’t been updated- they changed the color of the turf from green to black.
-They really focus on training and their trainers from what I have seen are professional, coherent and educated. Rates are very reasonable compared to what I personally charge and other gyms I have gone to charge (when comparing trainers – styles, professionalism, and actual training)
- Front desk is there from 8-1pm M-Sat. The other entrance is a 24hr entrance accessed with fob and has security cameras linked to police station.
- Kids Center – 30 min intervals – have to call in advance to get a spot, was told to be safe 48 hours in advance. I have kids and Elite and other fitness centers were nice that you could drop them at any time.
- 2 studios: 1 cycling studio (smaller than I am used to seeing, but up for debate) and 1 large group fitness studio.
- If you are looking for young energetic Classes – I am not 100% this place is for you. But that is up for you to decide- recommend a couple trial passes to see.
- Small Group Training is extra and that is done on the workout floor around the sprint track.
- The height of the building helps, but the turf area gets filled quickly and equipment isn’t placed in the most convenient places. It feels like you must maneuver around people training a lot of the time to get items from the corners.
-Locker Rooms: Smaller lockers and locker room - fills quickly, digital locks (no need to bring your own), dry sauna (cramped entrance but can fit 3-4 comfortably, and 3 or 4 shower stalls. I didn’t see they hand out towels or told they do, but see people bring their own.
They do not have: Steam rooms, indoor or outdoor pools, tennis/pickleball courts. Grafton is their main fitness location with a training studio in Mequon.
-For the time being, it is valuable to me – 2 min from my house, better fitness energy and environment than Elite Mequon and River Club, if choosing between these. The price point is very reasonable for what they provide. They are a local fitness focused gym, if you need more than that don’t waste your time- go to River Club and Elite for a tour.
