In 2006 Form & Fitness Health Club opened Wisconsin’s FIRST speed, agility, and strength training facility for athletes. This state of the art facility features a certified training staff as well as the latest and best training equipment for strength, function, and performance enhancement training.

Had to come back and edit my review from 2 years ago. It is best in the area, but that is not saying much, given there is little to compete. This place has 5 star POTENTIAL. Particular staff members and management keep this place from being elite.

I love this place! Clean facility with amazing staff and any machine you could need. Personal training is available but if you feel like being sociable there are classes galore. And to top it off their post workout protein shakes are amazing! If you’re there make sure you ask Mike at the counter for a protein builder chocolate PB and banana shake. Tell him Adam said that he makes them the best 😉

I joined Form and Fitness in Grafton about 2.5 months ago now and I absolutely love it! Mike at the front desk was super helpful when I signed up and continues to be a bright, friendly face every day when I come to work out (and he makes the BEST protein shakes every day)! If you’re looking for a gym with friendly members, awesome shakes, a sauna, tanning, personal training and ALL the things, join this gym! You will not be disappointed!! 💕

The form and fitness facility is the best gym I have found. They always have friendly staff that greet you and the equipment is always up to date.The gym is pristine all the time. I always have a positive experience while training and the sauna is a great addition that I can add to my routine. And the protein shakes are FANTASTIC! Request for Mike to make your shake. The shakes he makes are always consistent and delicious!

- I have moved gyms a lot because of location and experience – XSport, Equinox (Chicago), Lifetime Fitness (Brookfield), and Elite (all locations). I hope the information I provide you with helps with your decision.
- Energy and visual is nice. If you like to get some work done at the gym there isn’t a quiet space to do so, it is more of a café setting seating area with louder music.
-Pictures posted on here haven’t been updated- they changed the color of the turf from green to black.
-They really focus on training and their trainers from what I have seen are professional, coherent and educated. Rates are very reasonable compared to what I personally charge and other gyms I have gone to charge (when comparing trainers – styles, professionalism, and actual training)
- Front desk is there from 8-1pm M-Sat. The other entrance is a 24hr entrance accessed with fob and has security cameras linked to police station.
- Kids Center – 30 min intervals – have to call in advance to get a spot, was told to be safe 48 hours in advance. I have kids and Elite and other fitness centers were nice that you could drop them at any time.
- 2 studios: 1 cycling studio (smaller than I am used to seeing, but up for debate) and 1 large group fitness studio.
- If you are looking for young energetic Classes – I am not 100% this place is for you. But that is up for you to decide- recommend a couple trial passes to see.
- Small Group Training is extra and that is done on the workout floor around the sprint track.
- The height of the building helps, but the turf area gets filled quickly and equipment isn’t placed in the most convenient places. It feels like you must maneuver around people training a lot of the time to get items from the corners.
-Locker Rooms: Smaller lockers and locker room - fills quickly, digital locks (no need to bring your own), dry sauna (cramped entrance but can fit 3-4 comfortably, and 3 or 4 shower stalls. I didn’t see they hand out towels or told they do, but see people bring their own.
They do not have: Steam rooms, indoor or outdoor pools, tennis/pickleball courts. Grafton is their main fitness location with a training studio in Mequon.
-For the time being, it is valuable to me – 2 min from my house, better fitness energy and environment than Elite Mequon and River Club, if choosing between these. The price point is very reasonable for what they provide. They are a local fitness focused gym, if you need more than that don’t waste your time- go to River Club and Elite for a tour.

I moved to the area just under a year ago and was looking a new gym. I met with Mike and he showed me around. I gave the gym a try and signed up shortly afterwards. The staff is super nice. The equipment is great and the people working out here a friendly. It is a great gym and I’m glad that I joined just over a year ago. Plus I love an Arnold shake every so often after a good work out. I highly recommend this gym to anyone looking for a place to fit in.

Started working out at the Grafton location about a month ago excellent facility, staff is very friendly, everything you need is here. Weights, cardio options, modern equipment ,workout rooms plenty of space. Used to be a member at Elite in Mequon didn’t even know this place existed, glad I made the change.
