Small Group Personal Training

Not everyone needs 1-on-1 training and not everyone feels comfortable in a bigger group. For those who fall in-between, we offer group training. In an intimate group of just a few people, you’ll get group instruction with plenty of individual attention.

Athlete Training

In 2006 Form & Fitness Health Club opened Wisconsin’s FIRST speed, agility, and strength training facility for athletes. This state of the art facility features a certified training staff as well as the latest and best training equipment for strength, function, and performance enhancement training.

Form & Fitness offers signature strength and speed training classes for young athletes and 1-on-1 personal training, in addition to team training, speed camps, as well as educational clinics for athletes, coaches and parents.

Physical Therapy

CALBody Renovation Physical Therapy, S.C. is Ozaukee county’s premiere sports medicine and orthopedic rehabilitation center, offering a combination of manual therapy and orthopedic exercise in the treatment of orthopedic injuries. Located within Form & Fitness Clubs in Grafton and Mequon, Body Renovation is a full-service therapy clinic open to the public, working with patients of all ages and activity levels.

Nutrition Planning

Our most popular weight management program takes just 8-weeks, where you learn real world strategies that will transform your life!

Along with your nutrition classes, you’ll participate in an exercise program created by one of our knowledgable fitness professionals. You’ll get all the support and guidance through this journey to make your transformation a success!

I have been going to training with Tom for 11 years! He has gotten me through a torn rotator cuff, two knee replacements and a broken wrist. This was very personalized and modified for me to be safe and ease me back into my regular workouts. I also used the professional physical therapy area of form and fitness for my first knee rehabilitation. Everyone working there is professional and fun and Tom is of course a huge reason I’m as strong as I am at 64.

The experience at Form and Fitness is a cut above the rest. All of the trainers are certified and several have degrees in kinesiology. Every trainer is client focused and attentive to the ability, safety, comfort and goals of each client. After baby number two, I started doing small group training with Alex at the Mequon location. It has been such a positive reentry to my fitness journey! Alex makes sessions equal parts fun and challenging. He knows when to push me, but always has an eye on my form. I love that the groups are capped at 4 - enough for some camaraderie but still personal. Highly recommend if you want a local gym with fitness pros dedicated to your success.

This is the premier personal training studio in the North Shore. All the trainers are first class. Each trainer makes sure your workout is individualized to you and always look out for your welfare as you work out. The facility is always clean, and everyone is very welcoming.

Woodie Adkins here. I have been in the mequon facility +/-10 years. I especially like the size and fact that it is not a public gym, but utilises trainers for each client. My trainer is Josh Reisiner, an educated, very professional individual , respectful of time, flexibility, in scheduling and sensitive to my personal objectives for my 82 year old body and my stubborness to maintain condition after a heart valve and 4 by passes in 2022. Josh is my mentor and 56 years +/- my junior.

Enjoyed doing personal training with Jessica at Form & Fitness. It was very easy to get started with initial meeting, initial workout and weekly workouts together for the past 6 months. Would highly recommend Jessica and the location!

I work out 2x per week at the Mequon location for 1/2 hr each time - and I am 67! My personal trainer is Alex. I have been doing this now for over 3 years - and I keep coming back. The entire staff is great and everyone knows your name. We have a lot of laughs. Psst! and don;t tell the owner Ben that Alex really is pretty great!

Susanne has worked us hard during our weekly 30 minute sessions, but she's been very fair in not overdoing our workouts. She understands our personal goals (both stated and unstated) and is knowledgeable about our issues with Parkinson's disease.
