TRAINING AND SERVICES
Form & Fitness is not just another personal training studio. Founded in 2001 by a team of licensed physical therapists and certified personal trainers, we offer a unique perspective on getting you the results you want in the safest, most effective format possible.
ONE-ON-ONE
PERSONAL TRAINING
if you want a thoughtful and appropriate health and fitness plan, then you want to work with people who are passionate about fitness and about learning — that’s us!
We are not a gym. So, we won’t “boot-camp you into shape.”
In fact, we put your safety first. Your trainer will do a thorough assessment of your fitness readiness, exercise history and experience — and what you want to achieve with your health and fitness.
From there, your trainer will create a customized exercise program just for you and your unique goals and needs.
At every visit, your trainer will carefully observe your capabilities, muscle strength and weakness, range of motion and pay special attention to the safety of each and every movement during your workout. That means you’ll be working on the right activities so you can make progress quickly and safely.
SMALL GROUP
PERSONAL TRAINING
Form & Fitness offers signature strength and speed training classes for young athletes and 1-on-1 personal training, in addition to team training, speed camps, as well as educational clinics for athletes, coaches and parents. We deliver a positive training experience where all athletes in our program will become stronger, faster, and more confident! The goal of our program is not only to improve athletic ability by means of speed, agility, strength, flexibility, and injury prevention; but also developing character, confidence, accountability, and discipline.
We believe the emphasis should be on a foundation or strength, speed, endurance, flexibility, coordination, self confidence, and then sports specific skills and knowledge.
PHYSICAL THERAPY
CALBody Renovation Physical Therapy, S.C. is Ozaukee county’s premiere sports medicine and orthopedic rehabilitation center, offering a combination of manual therapy and orthopedic exercise in the treatment of orthopedic injuries. Located within Form & Fitness Clubs in Grafton and Mequon, Body Renovation is a full-service therapy clinic open to the public, working with patients of all ages and activity levels.

This includes development of individualized patient treatment plans emphasizing practical, functional, and direct, hands-on treatment from highly trained and skilled therapists. I make it a goal not only for people to have a challenging workout, but also to make my training sessions as fun and entertaining as possible. About me
The health and wellness field has always been of interest of mine dating back to Freshman year of High school. I joined my first gym in 1998 where I began lifting weights and became interested in how the human body works.
While I have been working out regularly since then, I actually began my fitness career working as a Membership Lead at the front desk in 2017. I loved my experience at the desk as it gave me an opportunity to motivate, encourage, and inspire new and existing members.
Realizing my love for fitness and motivating people, I knew it was time to further my career by becoming a Personal Trainer. I am proud to say I am now an ISSA Certified Personal Trainer, and cannot wait to change your life. My areas of expertise include: weight-loss, muscle hypertrophy, functional fitness, and nutrition.

Mikkel Kotila
About me
Health, fitness, and overall wellness have been my focus for the past 8 years, but fitness has been a huge part of my life since I was very young.
It all started with my mom and dad instilling the joy of sports into mine and my brothers’ lives. Around six or seven years old, my brother Erik and I began to see the importance of strength and fitness and how it directly applied to how we performed in sports. We even created our own (very basic) workout program consisting of mostly pushups, pull-ups, sit-ups, running, and hundreds of bicep curls. Before we knew it, we were maxing out some of our school’s physical fitness tests. Comical, yet amazing to reflect on how far back fitness and health goes for me! Fast forward 12 years and Erik introduced me to the sport of CrossFit.
This sparked a fire inside of me that spurred a whirlwind of fascination in health and wellness. That fire in my heart constantly pushes me to pursue excellence in everything I do, especially nutrition, mental resilience, and fitness. With all of that said, these reasons are exactly why I am SO excited to be a trainer and coach at Form and Fitness, surrounded by an amazing team of elite-level trainers, coaches, therapists, and staff.
Prioritizing health and fitness improved my life to levels I did not know existed. Now, it is my dream to help as many people as I possibly can; showing and leading by example, how dialing in nutrition and exercise can produce the best version of yourself.
See you out there!

About me
Exercise IS for everyone. No, it is not easy, if it were, everyone would be healthy and happy. But it’s not impossible, or no one would be. Exercise is not limited to lifting weights or running on a treadmill for hours, options for exercise are limitless. It is structured play and can be fun! I have a degree in Kinesiology and Psychology, with a focus on Exercise Physiology, Biomechanics, and Nutrition. I am also an ACSM certified personal trainer. I have 3 years of personal, group, and virtual training. Long story short I can confidently say I know what I am doing. Beyond the science, my philosophy of exercise is that it can and will be life changing if you let it. My goal as a trainer is to not only be an example of a life changing transformation, but also teach you to be your own trainer.
I will help you achieve your goals and teach you how to train yourself along the way. My goal is not to be a need for you to excel, but a luxury to go beyond what you can do on your own. Anything is better than nothing, future you will thank you.

Abi Marcquenski
About Me
I have been in a health and fitness setting for most of my life. Playing 3 sports through high school, up to college, and my parents running their own fitness facility for over 10 years, it was almost inevitable that I would become passionate about making a career out of the exercise science field. That being said, I have had very rewarding experiences working with the general population who want to shed a few pounds, improve their overall quality of life, move around pain free again, or improve their physique, as well as, working with athletes who want to become more successful in their sport.
Throughout my years in the gym and working with other fitness professionals, I have found and learned that consistency, self discipline, and balance is THE secret to reaching and sustaining any and all goals. My job will be to broaden your knowledge, help you stay consistent, remain injury free, but most importantly, to ENJOY THE PROCESS.

Bachelors in Exercise Science at the University of Wisconsin Parkside

About me
Fitness has always been important to me. Throughout my childhood it was my dad that got me moving. Whether it was mowing the lawn, going for long hikes or bike rides, he was the one who inspired me to be where I am today. Sometimes we need a push from another person to maintain our drive and get us on our feet each day. Today, I continue the hobbies that got me moving. I love to waterski, barefoot ski, hike and work out to maintain my overall fitness.
I want to be the one to inspire you like my dad did me. I want to be the one to push YOU. As your personal trianer, I will be here to motivate and educate you to be successful. From flexibility and core strength to functional training and bootcamps, I will help you achieve YOUR goals. Overall health and well-being require dedication and commitment. It’s a lifestyle.
After attending a four-year program at the University of Wisconsin-Milwaukee, I love that I continue to learn while I teach others. With a Bachelor of Science in Kinesiology (the study of human movement sciences), it is rewarding to be able to apply my knowlege and help others reap the rewards.

I have been going to training with Tom for 11 years! He has gotten me through a torn rotator cuff, two knee replacements and a broken wrist. This was very personalized and modified for me to be safe and ease me back into my regular workouts. I also used the professional physical therapy area of form and fitness for my first knee rehabilitation. Everyone working there is professional and fun and Tom is of course a huge reason I’m as strong as I am at 64.

After baby number two, I started doing small group training with Alex at the Mequon location. It has been such a positive reentry to my fitness journey! Alex makes sessions equal parts fun and challenging. He knows when to push me, but always has an eye on my form. I love that the groups are capped at 4 - enough for some camaraderie but still personal. Highly recommend if you want a local gym with fitness pros dedicated to your success.

Woodie Adkins here. I have been in the mequon facility +/-10 years. I especially like the size and fact that it is not a public gym, but utilises trainers for each client. My trainer is Josh Reisiner, an educated, very professional individual , respectful of time, flexibility, in scheduling and sensitive to my personal objectives for my 82 year old body and my stubborness to maintain condition after a heart valve and 4 by passes in 2022. Josh is my mentor and 56 years +/- my junior.

Susanne has worked us hard during our weekly 30 minute sessions, but she's been very fair in not overdoing our workouts. She understands our personal goals (both stated and unstated) and is knowledgeable about our issues with Parkinson's disease. We are at Form & Fitness thanks to a recommendation from a friend, and we have already passed along similar recommendations to others.
